Job Description
As a Direct Service Worker (DSW) with HHFHH, you will provide compassionate and personalized care to clients in their homes. Your role will involve assisting clients with daily living activities, personal care, and ensuring a safe and supportive environment. You will work closely with clients, their families, and healthcare professionals to deliver quality care that meets Medicaid guidelines and enhances the client's quality of life.
Quailifications
Education: High school diploma or equivalent required. Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
Experience: Previous experience in home care, healthcare, or a related field preferred.
Skills: Strong communication, interpersonal, and organizational skills. Ability to work independently and as part of a team.
Physical Requirements: Ability to perform physical tasks such as lifting, transferring, and assisting clients with mobility.
Other Requirements: Valid driver's license and reliable transportation. Must pass a background check and drug screening
Responsibilities
Personal Care: Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility.
Household Tasks: Perform light housekeeping duties such as laundry, meal preparation, and maintaining a clean living environment for clients.
Medication Assistance: Remind and assist clients with medication administration according to the care plan.
Transportation: Provide transportation or accompany clients to medical appointments, errands, or social activities as needed.
Companionship: Offer companionship and engage clients in meaningful activities to promote mental and emotional well-being.
Observation and Reporting: Monitor clients' physical and mental health and report any changes or concerns to the supervisor.
Documentation: Maintain accurate and timely documentation of care provided, including progress notes, incident reports, and client logs in compliance with Medicaid and company policies.
Compliance: Adhere to all Medicaid guidelines, state regulations, and company policies regarding client care and safety.
Communication: Collaborate with clients, families, and healthcare teams to develop and implement personalized care plans.
