Community Choice Waiver

The Community Choices Waiver (CCW) offers an alternative to nursing home care for seniors and adults with disabilities that develop later in life. Recipients undergo a standardized assessment to determine their care needs, and a budget is allocated accordingly to design a personalized service plan. The CCW provides a comprehensive range of services, including support coordination, nursing and therapy assessments, in-home monitoring, home modifications and assistive technologies, personal care, home-delivered meals, and respite care for caregivers.
